


The goal of this work stream is to ensure that all individuals within the area of the Network who have experienced a stroke or TIA are provided with the appropriate assessment and subsequent hospital treatment and care, following the acute stroke phase and in accordance with the National Stroke Strategy.
In light of this, the Network aims to:
Promote efficient and appropriate referral for acute stroke care and rehabilitation following the initial acute stroke assessment once the patient has been repatriated from the CSC and PSC
Develop an effective model for IHT stroke and TIA transfers from CSC and PSC to DSC
Engage hospital teams to improve services for stroke and TIA patients
Improve access to diagnostics across Greater Manchester and Cheshire for stroke and TIA patients
Increase the proportion of stroke patients spending all their time in hospital on a stroke unit across the conurbation
Demonstrate improved outcomes in relation to long term functional outcomes for stroke patients on discharge
Increase patient and carer involvement with local stroke service improvements
Engage staff to ensure we have a competent workforce across the pathway for stroke
The measurements for success will be:
Reduction in stroke mortality and morbidity and long term functional disability
Improved equity and access to stroke units across the Network
Increase in the number of stroke patients spending time on a stroke unit
Increase in the number of stroke patients being discharged to their home address, as opposed to placements in nursing/residential care
Numbers of healthcare professionals attending Network courses and training sessions
