The goal of this pathway is to ensure that all patients who experience chest pain are able to access timely clinical assessment by a chest pain specialist and have appropriate diagnostic imaging. In addition, clinically appropriate patients will benefit from best practice interventional procedures, and will be able to take advantage of local menu driven services in their rehabilitation. Finally, acute MI patients will have access to gold standard emergency care throughout the Network.
In light of this, the Network aims to:
Expand wireless telemedicine in primary care, including ECG monitoring and interpretation
Redesign services across the Network in order to deliver 24/7 access to Primary PCI
Ensure equitable access to Rapid Access Chest Pain Clinics and develop chest pain services
Ensure patients are on the correct pathway of care for chest pain
Improve transfer of digital diagnostic images across the Network
Improve access to functional imaging services with interpretation within 2 weeks
Be compliant with guidelines for angiography for ACS
Expand the diagnostic workforce
Support and improve fitness for low and intermediate risk patients
The measurements for success will be:
24/7 PPCI service for all Network health economies
Increase in activity of functional imaging
Increase in RACPC activity
Increase in cardiac rehabilitation activity
