


The Greater Manchester & Cheshire Cardiac and Stroke Network have a dedicated team who focus their time on working to improve the cardiac services for patients within the local geographic region. This work programme is driven from the local Cardiac Strategy which summarises the strategic goals of the Greater Manchester and Cheshire Cardiac and Stroke Network for 2008 - 2011. The strategy is developed through a process of review and consultation with key clinical, managerial and patient representatives from the cardiac community. The work programme defines work streams, objectives and deliverables for each stream and defines the expected benefits on successful delivery. These work streams include:
Chest pain pathway - which aims to ensure patients who experience chest pain are able to access timely clinical assessment by a chest pain specialist and have appropriate diagnostic imaging.
Breathlessness pathway - aims to ensure patients will have access to diagnostic imaging and specialist cardiology heart failure clinics, with on going care delivered through an integrated approach.
Adults with congenital heart disease pathway - aims to implement the agreed model of care for a specialist adult with congenital heart disease service as set out in the DH guidance.
Cardiac rehabilitation pathway - aims to expand cardiac rehabilitation access for all pathways; chest pain, breathlessness and palpitations and promote a menu based approach to the cardiac rehabilitation service.
