


The goal for this pathway is to implement the agreed model of care for a specialist adult with congenital heart disease (ACHD) service within Northwest England and North Wales and as set out in the DH guidance (May 2005).
The Network aims to:
Implement the agreed model of care for a specialist ACHD service within Northwest England and North Wales within resources made available by commissioners
Clarify and understand patient flows (current demand and forecasted future demand)
Identify gaps and shortfalls in provision, and develop investment plans for 2008 – 2011
Identify specific requirements of the hub and spoke centres (including geographic location) and develop service specifications. Support the provision for the infrastructure required for this
Support the 3 year Workforce Plan to address the projected high workforce requirements and associated costs
Develop and implement protocols and guidelines for the management of ACHD
Develop and implement a Communication Strategy/Plan to support implementation across the Northwest and develop a range of information/education materials
Establish an evaluation/audit programme to assess impact of service model/post-implementation. Review and identify activity during the 3 year implementation in order to assess gaps and inform the implementation process
Develop an ACHD database
Success will be measured by:
Increase in activity at the hub and spokes
A decrease in the numbers of patients being treated in undesignated centres
